Instructions:

Complete this form and press "submit" to request an application (required) and absentee ballot for the February 5th, 2008 General Primary Election.

By statute, the County Clerk’s office may not mail ballots more than 40 days or less than five days prior to the election.

Important: Returned applications and voted absentee ballots by mail must be postmarked by midnight on the day before the election and received within 14 days after the election.
